AB2BC Healthcare is inviting a small number of US medical practices to pilot home blood-pressure monitoring with a virtual care team — working alongside the practice’s existing electronic health record, not replacing it.

What a pilot practice gets

  • Home blood-pressure monitoring: patients send readings from their phone and get a clear next step on every reading.
  • A nurse care team that reviews alerts, calls patients and logs care time for the practice.
  • Summaries and alerts inside the practice’s EHR, and draft notes its clinicians edit and sign.
  • Private video visits, care plans and tracked labs and referrals when the practice chooses to add them.
  • Patient and care-team screens in 25 languages.

How a pilot starts

Each pilot begins with a written agreement, a connection to the practice’s EHR, training for its team and an agreed emergency pathway. The practice’s physicians stay in charge and choose which patients to invite. Every step can be paused or reversed.

Practices: ask about the pilot. Licensed doctors: join as a verified provider — NPI, state licences and the federal exclusion list are checked before you see patients.

AB2BC Healthcare provides software and care-team services to licensed medical practices; the practice remains the healthcare provider. Not an emergency service — in an emergency, call 911.
